Electronic On-Line Registration

1.         Signup - Create an account and register

2.            Upload a recent head-shot picture of yourself to be used on the Player ID Card

3.            Read and Check off the Waiver box and Pay on-line registration fee

4.            Email of copy of your Driver's License to ISSA Registrar for identification for age verification **

5.            Once Payment is received the team manager will recieve a temp player ID Card by email to be used for upcoming game.

6.            Plastic permanent ID Cards will be mailed out once payment is recieved.

7.            Temp ID Cards are only valid for 7 days from date shown on pass.

 

Manual Registration

1.            Download Player Registration Form from

2.            Fill out form completely including email address (Highly recommended)

3.            Send registration form to Illinois State Soccer Association (mail or email)

4.            Send Payment by check or money order ($22) with registration form.

5.            Player Pass will be mailed to Team Manager before season start date

 

** Accepted Legal forms of Identification:  State issued Drivers License, State ID, Passport
